Youth Support

What’s on UR Mind’ is a Targeted Youth Support pilot scheme running across Lancashire. It is open for referrals from Monday 3rd October 2011.

The aim of this project is to increase the mental, physical and emotional wellbeing of young people across Chorley, South Ribble, Preston, Fylde and Wyre, specifically targeting the most vulnerable young people who may be disadvantaged by circumstances, disability and mental health issues.

We will provide one to one engagement and assessment for each young person referred or self referred into the project focussing on the five areas identified by

Every Child Matters:

- Being Healthy
- Staying Safe
- Enjoying and Achieving
- Making a positive contribution
- Economic Wellbeing

After initial assessment, a key worker will be identified and the one to one support would continue according to assessed need. Sign-posting and support to access community services, counselling, physical activities and other educational/training opportunities will be offered in accordance with needs and goals identified by the young person.

In addition a variety of groups could be delivered dependant on recognised need which could include; WRAP (Wellness Recovery Action Planning); Self Esteem and Confidence Building Course; Anger Management course (Six - 2 hour sessions, for 6 - 8 clients); or an introduction into Mental Health Awareness (2 hours).

WRAP is an internationally recognised wellbeing and recovery action plan programme that has a positive impact on self esteem and confidence. It will improve longevity and maintaining wellbeing into adulthood, It is ideally suited to young people, encouraging development of a personal plan of self help strategies through discussion and insight. It offers positive, individualised, outcome-focussed life skills training. It can also be adapted for people with learning difficulties without diluting the impact of the programme.

We have extensive evidence of the effectiveness of WRAP and all the courses offered will provide skills by developing coping strategies and managing emotional health and wellbeing. Introducing this to young people will provide them with skills for life.

To be eligible for this service, individuals must fall within two of the groups below


• Not in education (13+), employment or training, or at risk of becoming so;
• Looked after by the local authority, at risk of becoming so or those leaving care;
• Engaging in risk-taking behaviour which may be harmful to their health and wellbeing;
• Involved in the criminal justice system or at risk of involvement;
• Have Learning Difficulties or Disabilities, including a physical impairment;
• Already a young parent or at risk of unplanned early parenthood;
• Young people who are homeless/at risk of being so;
• Young carers;
• Young people from most deprived wards;
• Young people living in isolated/rural communities;
• Black and Minority Ethnic young people (including Gypsy/Roma/Travellers):
